The Isimu–Paguyaman Road is a national road connecting Gorontalo and Central Sulawesi Provinces and plays an important role in supporting regional transportation and economic activities. The road section has a pavement width of 2 × 3 m and consists of both flexible and rigid pavement. Increasing traffic volume has resulted in various pavement distresses, necessitating an evaluation of road conditions using the Pavement Condition Index (PCI) method. This study aims to identify the types of pavement distresses, determine the pavement condition based on the PCI method, and formulate appropriate maintenance alternatives. The results showed that 11 types of pavement distress were identified, namely alligator cracking, depression, edge cracking, shoulder drop-off, longitudinal/transverse cracking, patching, aggregate polishing, potholes, rutting, slippage cracking, and weathering and raveling. Based on observations of 100 sample units, the average PCI value was 79.30, indicating that the pavement condition was categorized as Very Good. Therefore, routine maintenance is recommended to maintain pavement performance and extend the service life of the road.
